And is there a $PublicAccess field on the form? The Form may be available to public access users, but the document it creates will not be a "public access " (and therefore anonymously writable) document unless it contains a $PublicAccess field with a value of "1". The user doesn't get an error message -- the document just doesn't save.

(I noticed this behaviour -- the no errors thing -- after moving people from Author access to No Access and back -- the "Create Documents" item does not automatically check when you do that. Users receive no errors and can edit anything they'd normally be able to, but can't save new documents. Fixing needs extra eyes, cause you'll never see that check mark through the gathering red mist....)
